        Medilly assisted reproduction and the risk of preterm birth: a se–control study using data from the Quebec Pregnancy Cohort

        The use of fertility treatments has been growing over the past dede, but these treatments are not without risk. In this se–control study using data from the Quebec Pregnancy Cohort (2010–2015), the authors quantified the risk of preterm birth associated with the use of ovarian stimulators (OS) and assisted reproductive technologies (ART). During the study period, 2055 pregnancies out of a total of 57?624?pregnancies were conceived through the use of OS, ART or both: 419?involved OS alone, 150?involved ART alone and 1486?involved both OS and ART. Adjusting for potential confounders, conception with OS, ART or both was associated with an increased risk of preterm birth (adjusted odds ratio 1.46, 95% confidence interval 1.25–1.72, 182?exposed ses). All types of assisted reproduction were associated with an increased risk of preterm birth compared with pregnancies conceived spontaneously. Use of OS or ART or both was associated with an increased risk of late, moderate and extremely preterm birth. Physicians should advise patients of the increased risk of preterm birth associated with the use of fertility treatments so that they n make informed choices. Full article

        Diabetes during pregnancy and perinatal outcomes among First Nations women in Ontario, 2002/03–2014/15: a population-based cohort study

        This study examined temporal trends in pregnancy outcomes and use of health re services in a population-based cohort of First Nations women compared to other women in Ontario from 2002?to 2015. First Nations women had a higher prevalence of preexisting diabetes and gestational diabetes than other women in Ontario. They also had a higher rate of preeclampsia, labour induction and cesarean delivery. The rate of preterm birth was similar between First Nations women and other women in Ontario. Although First Nations women had a higher rate of babies who were large for gestational age than other women, regardless of diabetes status, obstructed labour rates were similar for the 2 cohorts. Almost all First Nations women, regardless of diabetes status, were seen by a primary re provider during their pregnancy, but rates of use of specialty re were lower for First Nations women than for other women. Fifteen percent of all pregnant women with preexisting diabetes visited an ophthalmologist during their pregnancy. Full article

        Knowledge, attitudes and practices of nadian pediatric emergency physicians regarding short-term opioid use: a descriptive, cross-sectional survey

        In the midst of the current opioid crisis, physicians are ught between balancing children’s optimal pain management and the risks of opioid therapy. In this study, 136?(60.7%) of 224?physicians in the Pediatric Emergency Research nada database responded to a survey about their practice patterns for prescribing, knowledge and attitudes regarding, and perceived barriers to and facilitators of short-term use of opioids. Intranasally administered fentanyl was the most commonly selected opioid for first-line treatment of moderate (47 respondents [34.6%]) and severe (82?[60.3%]) pain from musculoskeletal injury. The respondents appeared minimally concerned about physil dependence, addiction risk and the current opioid crisis affecting use of opioids in the emergency department or at home. The top 3?reported barriers to prescribing opioids were parental reluctance (57?[41.9%]), lack of clear guidelines for pediatric opioid use (35?[25.7%]) and concern about adverse effects (33?[24.3%]). Evidence-based development of guidelines and protocols specific to prescribing opioids in children may improve physicians’ ability to manage pain in children responsibly and adequately. Full article
